- Polyarticular, symmetric arthritis (60% of cases) involving peripheral joints, including the hands (MCP, PIP, and wrist), knees, and ankles (mimic rheumatoid arthritis)
- Other nonspecific findings include fever, fatigue, and diarrhea
- May have nonspecific morbilliform rash or no skin symptoms at all (rather than classic slapped-cheek rash in children)
- Diagnosis of acute infection is with anti-B19 IgM antibodies (IgG = previous infection)
- Symptoms typically resolve spontaneously in 2-3 weeks without the need for specific treatment
