Rationale for change

Per AMBOSS, Chronic ITP is platelet count < 100,000 for >1 year with no identifiable cause (primary ITP) or secondary to certain conditions (secondary ITP); I think leaving off the causes is fine since that's not the point of the card but then I think we need to remove 'is defined as' because it's an incomplete definition
