Rationale for change

The first delted part has no context at all, and with the deleted part in the extra "in this patient" shows that the card is made from a question about a patient with ALF due to hep B (mostly amboss question)

So i removed some parts and did some edits and i think we can shorten the extra more becasue there is a lot of unnecessary things there
