- Irregular 17 mm left occipital lesion with surrounding vasogenic edema
- T2 hypointense rim (suggesting hemorrhage/hemosiderin), central diffusion restriction
- Vivid peripheral contrast enhancement with finger-like projections and loculation
- Slightly elevated peripheral CBV
- MRS: prominent lactate/lipid peaks (necrosis), absent choline peak
- Differential includes GBM/metastasis, but restricted diffusion and lack of choline keep abscess (e.g. Aspergillus) in consideration—especially in immunocompromised patients
- Other findings: chronic small vessel disease; possible old cortical infarct
