Rationale for change

Removed prompt as it made the card too easy imo, there's other cards that test what is used for prophylaxis for PCP or toxoplasmosis in deck. Also clarified that prophylaixs should start <200 for all patients on ART.

Also updated tabe which said prophylaxis for toxoplasmosos is indicated for all patients which is wrong - UW and AMboss both say toxoplasmosis prophylaxis is only indicated for patients with positive serology
