Rationale for change

same question already tagged by (uworld 7553) has added a 3rd indication in the table; not sure if we want to just include in the extras or change card to say three indications here but it definitely needs included with the new uworld chart; the picture in the card already alludes to a 3rd criteria but slightly differs from the uworld one with the criteria after BMI > 30; uworld also refers to this as "bariatric (weight-loss) surgery" - this also makes the card more searchable in browser
