Some reasons:
1. This does not get tested where you have to point to where the anatomical lesion is on board exams
2. These cards in theory are very effective but the effectiveness is vastly reduced by having the answers in the photo, it reduces recall and makes it almost zero effort to answer some of the clozes, by removing the photo, learners are forced to know the underlying pathology. This will lead to better recall as well, this thing also gets tested alot in inhouse exams and these cards can be very good for this
3. The image will remain in extra, so if users want to learn the anatomical lesion its right there in extra, nothing got removed
