Proximal tibial nerve lesions present with:

- Foot {{c1::dorsi}}flexed and {{c1::everted}} at rest
- Loss of foot {{c1::inversion and plantarflexion::2}}
- Weakness of the {{c1::extrinsic}} muscles of the foot
Extra
- The proximal tibial nerve gives off branches that innervate the tibialis posterior (involved in foot inversion), as well as the gastrocnemius / soleus / plantaris muscles (involved in foot plantar flexion)

- Thus patients present with the foot held in the calcaneovalgus position (dorsiflexed / everted)

- Extrinsic foot muscles (e.g. flexor digitorum longus, flexor hallucis longus) are also innervated by this part of the nerve (resulting in loss of toe flexion)

- Proximal tibial nerve lesions (e.g. at the popliteal fossa) include knee surgery, baker cyst, and deep penetrating trauma
